Background

The Ministry of Health and the Ontario Medical Association have been working together to implement the 2024 Physician Services Agreement.

Following recommendations from the Physician Payment Committee, 2 new fee schedule codes (FSCs) have been established to recognize the physician time and clinical activities associated with the initiation and ongoing management of Food Allergy Immunotherapy (FAIT).

New FAIT fee schedule codes

Specialty eligibility

A620 and K621 are only eligible to specialists in Clinical Immunology (OHIP billing specialty ‘62’).

A620 service description

Initiation of Food Allergy Immunotherapy is an assessment service rendered by a specialist in Clinical Immunology, where the physician spends a minimum of 30 minutes (consecutive or non-consecutive) in direct, in-person contact with the patient. This is exclusive of time spent rendering any other separately billable intervention to the patient and is for the sole purpose of establishing and initiating:

• the FAIT treatment protocol (mono-allergen or poly-allergen), and
• initial therapy doses.

The physician must be physically present on-site and immediately available to intervene for the duration of the treatment session. If this requirement is not met, A620 is not eligible for payment.

Key payment rules for A620

• No other services listed under the ‘Consultations and Visits’ section of the Schedule of Benefits for Physician Services (schedule) are eligible for payment for the same patient, on the same service date as A620.
• Provocation testing (G208 and G190) is not eligible for payment for the same patient on the same date of service as A620.
• A620 is limited to a maximum of:
  • once per patient, per allergen, per lifetime, and,
  • once per treatment protocol, whether mono-allergen or poly-allergen therapy;
• Eligibility requires appropriate diagnostic evaluation supporting an IgE-mediated food allergy.

K621 service description

Food Allergy Immunotherapy (FAIT) support is a time-based service payable to a specialist in Clinical Immunology for providing direct in-person physician assessment, support, counselling, dose adjustment, and reaction monitoring of patients actively receiving FAIT.

This service is applicable to FAIT-related patient encounters following the initial dose-setting visit, provided the applicable time requirements are met.

Key payment rules for K621

• No other services listed under the ‘Consultations and Visits’ section of the schedule are eligible for payment for the same patient on the same date of service as K621
• K621 is limited to a maximum of:
  • 2 units per patient, per day
  • 12 units per patient, per physician, per 12-month period
• Time used to claim units for one patient cannot overlap with time used to claim units for any other patient.

Relationship to other insured services

Physicians are reminded that A620 and K621 were established to remunerate specific FAIT services performed in defined clinical circumstances. Medically necessary, non-experimental FAIT services remain insured under OHIP, regardless of whether the requirements of A620 or K621 have been met. Patients may not be charged for this care.

Where a service does not qualify for payment under A620 or K621, physicians may be eligible to claim other appropriate fee schedule codes, provided the applicable conditions for payment are satisfied.

Implementation timeline

Although the effective date of the FAIT fee schedule codes is July 1, 2026, the OHIP claims payment system will not be ready to accept claims at this time.

Claims will be submitted once system changes have been completed, which is anticipated in the fall.

Additional guidance on claims submission will be communicated as implementation progresses.
