Overview

Effective April 1, 2026, the Ministry of Health (the ministry) will use a single submission process, transitioning fully to electronic submissions in place of paper forms submitted by email or fax. Providers will be required to submit registration and maintenance requests through the secure, web-based application for the following services:

  • Provider Registration for an OHIP Billing Number
  • Health Care Group Registration
  • Consent and Authorization for Group Membership and Payment
  • Change of Address, Banking Information, and Group Information for Health Care Professionals or Health Care Groups
  • Application for IVR Participation
  • Social Assistance Verification Portal PIN Request

  1. The online platform improves accuracy through mandatory field validation and supports faster, more efficient processing.
  2. Electronic submissions enhance records retention, simplify document retrieval, and provide applicants with an immediate redacted copy of their submission.
