Overview
The Ministry of Health and the Ontario Medical Association have been working together to implement physician compensation increases in accordance with the 2024 Physician Services Agreement (PSA). The PSA provides Year 1 and Year 2 increases to be implemented as permanent adjustments to physician payments effective April 1, 2026.
As per INFOBulletin 260304 - Physician Services Agreement related fee schedule code adjustments, primary care agreements have been aligned with the new services as outlined below.
Changes to Core and Excluded services by contract type
Effective April 1, 2026, the following changes to Core and Excluded services have been made to the corresponding primary care contract types.
Service Type:
- Core service, are services also known as in-basket services that will pay the blended-fee-for-service rate for the contract type when provided to enrolled patients
- Long-Term-Care Core service, are services also known as in-basket services that will pay the blended-fee-for-service rate for the contract type when provided to Long-Term-Care enrolled patients
- Excluded service, are services also known as out-of-basket services that will pay fee-for-service when provided to any patient

Changes to Special Premiums
Effective April 1, 2026, the following services have been added to the corresponding Special Premium category.

Changes to After-Hours premiums by contract type
Effective April 1, 2026, ‘E089A – Gender add-on to K131 and K132’ has been added to the list of services eligible for all primary care after-hours premiums (Q012A, Q016A, and Q018A).
Changes to Family Health Group (FHG) Comprehensive Care premium
Effective April 1, 2026, ‘G849A – Respiratory Syncytial Virus (RSV) for patient’s first season’ and ‘E089A – Gender add-on to K131 and K132’ have been added to the list of services eligible for the FHG Comprehensive Care premium.
