The Ministry of Health has made changes to the language on the laboratory requisition for Prostate Specific Antigen (PSA) testing effective April 16, 2026.

Overview of changes effective April 16, 2026
The Prostate Specific Antigen (PSA) section on the current Ministry of Health Laboratory Requisition (Form #4422‑84) has revised language to support clinicians in ordering a PSA test. The language indicates whether the test is uninsured or insured under the Ontario Health Insurance Plan (OHIP) in accordance with the Schedule of Benefits for Laboratory Services under Regulation 552 of the Health Insurance Act.
The current language for insured testing states “Insured - Meets OHIP eligibility criteria”, which does not provide any information on the criteria.
The revised language for insured testing will elaborate on the criteria by stating “Insured – (i) suspected prostate cancer (family history, race, physical examination, symptoms); or (ii) diagnosed prostate cancer (monitoring/follow up)”
For uninsured testing, “Screening” will be revised to “Asymptomatic screening”.
Note: There are no changes to the OHIP-insured status of PSA testing. The revised language in Form #442-84 aligns with the current OHIP criteria for PSA insurability.
This change is effective April 16, 2026, and is reflected on requisitions that are embedded in OntarioMD certified electronic medical records.
