Update to payment rules for fee schedule codes E682A and K077A.

Background

The Ministry of Health and the Ontario Medical Association are implementing adjustments to physician payments effective April 1, 2026, as part of the 2024 Physician Services Agreement (PSA).

These adjustments are being added to the OHIP claims system to support recommendations from the Physician Payment Committee (PPC), specifically updating payment rules for Cardiac Surgery and Geriatric Medicine.

The following fee schedule codes (FSCs) will be affected by the change:

  • E682A - Pump bypass - graft of major vessel other than ascending aorta for the purpose of cardiopulmonary bypass or ventricular assist device
  • E650A - Pump bypass - includes cannulating and decannulating heart or major vein, major artery, supervision of pump and pump run
  • R701A -Ventricular assist devices - uni-ventricular
  • R702A - Ventricular assist devices - bi-ventricular
  • R703A - Ventricular assist devices - paracorporeal
  • R704A - Ventricular assist devices - implantable
  • Z743A - Circulatory assist device e.g. intra-aortic balloon – open
  • K077A - Geriatric telephone support

Payment rules for E682A

E682A was previously payable only when billed in conjunction with E650A. Effective April 1, 2026, E682A will be eligible for payment when submitted with E650A, R701A, R702A, R703A, R704A or Z743A for the same patient, on same date of service by same physician.

Payment rules for K077A

Effective April 1, 2026, the maximum number of units billable under K077A will increase from 8 units to 12 units per patient per 12-month period.

Medical Claims Adjustment

A medical claims adjustment (MADJ) will be run at a future date to adjust all impacted claims submitted between April 1, 2026 and November 1, 2026, to ensure they are paid in accordance with the revised payment eligibility conditions. Details regarding the MADJ will be provided in a future INFOBulletin.
